New Patients

At Your First Appointment:

Welcome to our office! We work very hard to make each new patient feel like a part of our dental family. At a new patient appointment, you can expect us to thoroughly discuss your medical and dental history, update radiographs and periodontal charting, and discuss any questions and concerns you have about your teeth.

New Patient Paperwork

All new patient health and dental histories, contact information, and consent forms are located below. You can complete the forms and email them to our office at [email protected] or print them and bring them with you to your first appointment!

Radiographs (X-Rays)

Our dental providers need up-to-date radiographs to help them properly treat you and your teeth. If you have had radiographs taken within the last six months at a different dental office, please let our staff know, so that we can attempt to retrieve those in advance. If you do not have recent radiographs, no problem! We’ll take all needed images at your first appointment.

Periodontal Treatment

After discussing your dental history and performing an exam, our dental team will discuss preventive and periodontal treatment with you. While some patients may need a “traditional cleaning,” other patients may need a more in-depth periodontal treatment plan, tailored directly to your needs.

Dental Insurance

We will file all forms of dental insurance. While we are only in network with Delta Dental Premier, we do receive reimbursement from almost every insurance company. As our fees are in line with most insurance companies’ allowed fees, we often find that patients do not see a difference in cost between in-network and out-of-network benefits at our office.

Medicare

We do not accept assignment of benefits for your Medicare or Medicare Supplement plans. While we will do all the hard work and file the claims for you, we will collect the cost of treatment at the time of your visit. Medicare will then directly reimburse you via check within 30 days of treatment.

Specialists

Sometimes there are dental procedures needed that are too complicated for a general dentist to perform. For those treatments, we work closely with a team of specialists in the Charleston area who share our same dedication to a positive patient experience. Should you need a pediatric dentist, oral surgeon, periodontist, prosthodontist, or endodontist, we can recommend and help you establish care at these wonderful offices.
